Information Needed

1 Type of Unit Are you a Pack or a Troop?
2 Number What is your Pack or Troop number?
3 City/Town What city or town is your unit registered in?
4 District What district is your unit registered in? Nathan Hale?
5 Date to Date What date are you leaving and what date are you returning?
6 Give itinerary / Destination Where are you going on this trip and what are you going to do?
7 Type of trip How long will you be there? (1 night = Touring Camp; 2 night = Short Term Camp; 3 night (72hrs or more) = Long Term. Camp)
8 Swimming/Boating/Climbing If swimming, boating or climbing is part of the program then you must have an adult with proper certification.  Fill in that person's information in the space.
9 Mode of Transportation How will you get there?  Check all that apply.
10 Tour will include How many scouts & adults are going on the this trip? (cub scouts require a 1 on 1 for camping OR if someone else, other than the tour leader, takes responsibility for 2 scouts).
11 Tour Leaders Name Who is responsible for this trip?
12 Tour Leaders Signature You have the Guide to safe scouting and have READ it.
13 Assistant Tour Leader Who on this trip is the tour leaders backup.
14 Signature - Committee Can be any committee member who is on your charter (MC, CC, CR).
15 Signature - Tour Leader Same person who is on line 11.

22 Dates to and from Same dates from line 5.
23 Total youth/adults Same amount as on line 10.

24 Insurance Who will be driving the scouts on the trip?  List all vehicles that will be on the trip and carrying scouts that are not related to the owner OR if the scouts are meeting at the trip location you can write "Parents Driving Own Scouts" across the box
